SITE VISIT INFORMATION
Grantees in the PHASE and TC3 programs will have the opportunity to visit a health center that exemplifies innovative population health management practices. Site visits will occur on the morning of Tuesday, December 3, followed by lunch and a learning exchange session at the Chez Shari Conference Center (Manteca, CA). To minimize disruption of clinic operations, space is limited to 20 participants across both sites. Each PHASE and TC3 grantee organization may register up to two staff members to attend.
On December 3rd, two organizations in the Central Valley will highlight promising practices and innovative approaches to improve care for patients at risk for cardiovascular disease. Through observation, demonstration, and deep discussion, visitors will gain first-hand insight into successes and lessons learned in both direct patient care and infrastructure development to support quality improvement.

Community Medical Center (Stockton, CA). CMC has 20 sites, with a PHASE population size of over 15,000 patients with hypertension, diabetes and/or ASCVD). Visitors will learn about how a large organization instills a quality mindset and skillset throughout the entire health center. We'll observe a "PDSA bootcamp", then do a deep dive into their diabetes shared medical visit model and finish by examining other important interventions - such as integrated behavior health - that provide comprehensive care for PHASE patients.
Who should attend? This site visit would be of particular interest to quality improvement directors and staff, diabetes care providers, care managers and coordinators.
Livingston Community Health (Livingston, CA). Livingston has 6 sites, with a PHASE population of almost 4,000. Visitors to Livingston will learn all about aspects of developing and implementing a self-measured blood pressure (SMBP) program. Patient education approaches and materials will be shared through an SMBP mock visit. After a clinic flow observation highlighting the role of health educators, Livingston will also share their approach to building a data driven culture through data governance aimed at improving data quality, increasing data literacy and maximizing data access.
Who should attend? This site visit would be of particular interest to providers and staff interested in SMBP, QI managers, and data analysts.
