3.During the reporting period did your Tribe conduct any exercise(s) through the PHEP Program not otherwise included in the BP4 work plan?
4.Since you answered "yes" to question #3, provide the name and date of each exercise conducted. Remember, within 120 of exercise completion an after-action report must be submitted to DEPR at MDHHS-BETP-DEPR-PHEP@michigan.gov.
