The purpose of this survey is to understand mental health and addiction lived experience networks in Aotearoa. It will add detail for the project to design a MH&A Lived Experience National Representative Body, led by Changing Minds with Balance Aotearoa, Te Kete Pounamu, and Thriving Madly. The project is funded by the Ministry of Health.
We want to hear from lots of different people about networks they belong to and utilise. This includes:
• tāngata whaiora (people who use or have used mental health and addiction services)
• whānau of tāngata whaiora (family member or support person of someone on a mental health or addiction journey)
• people who work in Consumer, Peer Support, Lived Experience (CPSLE) roles
• organisations that work in mental health and addiction.
The survey takes about 5-10 minutes. Your answers will be kept private. You can stop at any time. You can skip any question you do not want to answer. You must be 16 or older to take part.

Your privacy. All answers are kept private. Your answers will be grouped with others in our reporting. No individual will be named. You may choose to provide us with contact details for follow-up questions. Otherwise, your email address will only be used to keep you up to date about the project.
