Survey for Parents of Students Receiving Special Education Services in Central Unified School District |
Introduction
The Central Unified School District (CUSD) contracted with the WestEd Center for Prevention and Early Intervention (CPEI) to conduct a review of the district’s special education programs and services to identify strengths and provide recommendations for any areas of improvement. WestEd staff are conducting a comprehensive data gathering process and develop a final report with recommendations for improvement.
Feedback from parents of students who receive special education service is an important part of the process of evaluating any special education program. Please take the time to complete one survey for each child you have who is receiving special education services in CUSD. We need your input to provide to better understand the strengths of CUSD special education program and areas where the division needs to make improvements.
All of your responses will be kept confidential. Survey responses are returned directly to WestEd. The CUSD staff will not see any individual responses or raw survey data. Your responses will be used specifically for this research project and be used for the purposes of evaluating the special education programs and services in CUSD.
The survey will take approximately 15 minutes to complete. Please complete and return by January 27, 2017.
