The City of Stratford, Town of St. Marys, Municipality of North Perth, Municipality of West Perth, Township of Perth East, and Township of Perth South have developed a joint Community Safety and Well-being (CSWB) Plan intended to provide a roadmap for creating a more inclusive, connected, and coordinated approach to safety and well-being planning across all 6 municipalities. It consists of 4 priority areas:
  1. Systems planning and integration
  2. Adequate, affordable, and attainable housing
  3. Affordable and accessible health, social, and recreation services
  4. Social inclusion
The implementation of this Plan is the responsibility of a CSWB Partnership Council (CSWBPC) - made up of municipal and key stakeholder representatives - which will establish and oversee action groups to carry out activities related to the priority areas.

The first step in establishing these action groups is to understand what work is already underway and how that existing work aligns with the CSWB priorities. For this reason, the CSWBPC is conducting an inventory of local collaborative committees (defined as a group of agencies working together for a common purpose). The objectives of this inventory are to: 
  • Identify existing collaborative committees and assess their mandate and activities relative to the CSWB Plan
  • Understand how the CSWBPC can support the work of existing collaborative committees
  • Identify existing data sources and performance measurements that align with CSWB priorities
  • Identify gaps where new action groups may need to be established
The purpose of this survey is get a preliminary understanding of your collaborative committee and the work you are doing. This is intended to be a starting point for discussions about how the CSWBPC and your committee can work together to achieve mutual goals. 

The survey should take around 20 minutes to complete and can be done by one representative or by the committee as a whole. CSWB staff is also available to complete this survey with your committee in-person or virtually if that is more convenient.
