Welcome to the Counseling Service of Addison County (CSAC) Community Needs Assessment Survey

Wellbeing in Action, a health and human services consulting firm, on behalf of CSAC wants to learn more about Addison County needs related to mental health, substance use and developmental/intellectual disability needs. This survey helps us find out which services, like mental health and substance use programs or development services, are important to you and what might make it hard to get the help you need. By answering these questions, you're helping us make our programs better for everyone.
