Supporting healthy lifestyle choices through education, collaborative planning, and community 

Introduction:
This survey is intended to gather input from community members ages 18 and older about perceptions, concerns, and experiences related to substance use prevention, mental health, and overall community well-being.

Your responses will help identify community strengths, areas of concern, and gaps in resources or services. The information collected will be used to guide local planning, prevention efforts, education, and decision-making aimed at supporting youth, families, and the broader community.

Participation in this survey is voluntary, and your responses are anonymous. There are no right or wrong answers — we are interested in your honest perceptions and experiences.
