Will County COVID-19 Vaccine Viewpoints Community Survey

The purpose of this completely voluntary survey is to get more information from our community about their viewpoints on the COVID-19 vaccine and reasons why they chose to receive or not receive the vaccine. It takes no more than 5-10 minutes to complete. This survey is completely anonymous, and you will not be asked to provide your name or contact information, so this survey will not be connected to you in any way. Your personal responses will not be shared publicly. The information collected from this survey will help us better understand where our community stands on the vaccine so we can improve our outreach and education efforts, as well as address concerns that our community has about the vaccine. We would appreciate your response so we can better center our efforts to address COVID-19 infections and deaths in our county. Thank you!
