Diabetes Prevention Program Information:

Eligibility

You are not eligible for this program if you’ve been diagnosed with Type 1 or Type 2 diabetes.

You are eligible if you:
· are age 18 or older (not pregnant)
· have a BMI > 25 (calculate your BMI here)
· provide a LAB RESULT OR answer a QUESTIONNAIRE (below)

LAB RESULTS WITHIN THE LAST YEAR (only 1 required)
- Fasting glucose level of 100 to 125 mg/dl
- A1c level of 5.7 to 6.4
- Clinically diagnosed with Gestational Diabetes

I’ve always loved food, and I never paid that much attention to what I ate before. My father had type 2 diabetes, and he passed away… I started noticing tingling in my hands, tingling in my feet, and started thinking I might be getting type 2 diabetes, and I needed to make a change. My favorite thing about the video series [in the online lifestyle change program] was the folks were very similar to me. My coach was awesome, sending me emails telling me I was doing well.
