MDA Community Survey on COVID-19

As part of MDA's Frontline COVID-19 Response, MDA would like to hear from the community around the impact of COVID-19. This survey is to be completed by individuals with muscular dystrophy, ALS and related diseases and/or their caregivers. This survey is geared to help MDA better understand where there may be needs and gaps in information and support that MDA can prioritize. Please complete this survey by Wednesday, April 22.
