Before you begin the survey we want to make sure we have permission to use your responses. Please take a minute to read the statement below:
Participation in this survey is voluntary. You may choose not to complete the survey or stop the survey at any time. This information is being used to obtain new knowledge about resources and needs within Bladen, Robeson, and Columbus counties. You are being asked to complete the survey because you are a parent within the tri-county region. By choosing to complete this survey you are acknowledging that the Community Prevention Action Planning group can use your data to inform the creation of their plan.
The survey should take no more than 10 minutes to complete. Upon completion you will be given a separate link to enter your email to receive a $10 gift card (gift cards will be distributed to the first 100 respondents).
