CONSENT TO PARTICIPATE IN RESEARCH STUDY

KEY INFORMATION

You are being asked by Amy McClune to participate in a research study, taking part in the study is voluntary, and you may stop at any time.

The purpose of the study is to identify the characteristics of Registered Nurses during the COVID-19 pandemic.

In this study, you will be asked to complete a survey focusing on resilience, coping, spirituality and professional quality of life. It will take you about 15 minutes to complete the study.

The potential risks during the study are:
• Inconvenience for the time that it will take to complete the survey. 
• Revisiting situations that you have worked in over the past few weeks. At the end of the survey are suggestions for seeking assistance if needed.

There are no direct benefits to participants from the research however, you may find this an exercise in expressing your thoughts and feelings regarding your role in the COVID-19 pandemic. It will help me and other nurses better understand how this research relates to our profession.

The online study is completely anonymous; you will not be asked to give any information that could identify you (e.g., name). The survey is NOT linked to IP addresses. Individual responses will not be presented, just the aggregated data.

Remember, taking part in this study is voluntary. If while taking the survey you feel uncomfortable or no longer want to participate, you may stop at any time. To stop taking the survey, just close your browser. There are no consequences if you do not complete the survey.

This survey is anonymous. There are no records that will connect you with the information you provided. This consent is not signed and is not associated with the results of the survey. If you read this and decide not to participate, just choose “No, I do not consent” and you will be directed to an exit page.
The results of the study may be shared in aggregate form at a meeting or in a journal, but your personal information will not be revealed. Records from this study will be kept by Amy McClune, the principal investigator for a minimum of three (3) years after the study is complete.
