Introduction

The State of Indiana, as a recipient of funds from the U.S. Department of Housing and Urban Development (HUD), is conducting this survey for two purposes:

1)  To provide an opportunity for comment on the 2017 preliminary draft Annual Action Plan (Insert link here when available); and

2)  To confirm that the community development and housing needs identified for the 2016-2020 Consolidated Plan are still applicable.

This survey is meant for people who work in the fields of housing and community development, lending, social services, fair housing and advocacy. Because this study is focused on the state’s smaller cities and rural areas, we are especially interested in hearing from people who work in these communities.

We also welcome input on statewide issues that affect Indiana residents, regardless of where they live.

If you wish to provide feedback on all four sets of priorities (Community Development Block Grant, HOME, Emergency Solutions Grant and Housing Opportunities for Persons with AIDS), the full survey consists of 53 questions, BUT you may choose to provide feedback regarding only one set of priorities, in which case the survey would only be 12-20 questions that should take fewer than 10 minutes.
 
Your participation is voluntary, but very important to the development of a meaningful plan. All responses are confidential, and will only be reported in combination with those of other participants. Please invite others in your field to participate in the survey.
