Introduction

The San Diego County Childhood Obesity Initiative (COI) Community Domain, in collaboration with the University of California San Diego (UCSD) Center for Community Health, Community Health Improvement Partners (CHIP), the County of San Diego, and various community stakeholders, has created this survey to learn more about community engagement groups that empower and train residents to make positive changes in their neighborhoods. We are particularly interested in groups focused on increasing access to healthy foods and beverages, opportunities for physical activity, and safe routes to healthy places throughout San Diego County. As a valued partner, you have been invited to complete this survey on current and past community engagement groups.

The information from this survey will be posted on an online interactive map accessible to the public to serve as a tool to:

• Chart the collective ideas and networks surrounding community engagement efforts within San Diego County

• Connect peer-to-peer with other community engagement groups and residents working on similar focus areas or in the same places

• Inform ongoing projects and cross-collaboration

• Identify groups working to increase access to healthy foods and beverages, physical activity opportunities, and safe routes to healthy places.
