Welcome to the online application for the California State Library's Career Online High School (COHS) Program. This application is for new libraries to apply to participate in the Program. Current participants who wish to request more scholarships should use a separate application; contact Bev Schwartzberg (beverly.schwartzberg@library.ca.gov) for details.

PLEASE READ the Project Description and Requirements document available on the State Library's COHS program website (http://www.library.ca.gov/services/libraries/COHS.html) before completing this application; it contains information that may be helpful in developing your answers. You may wish to view the webinar archived at the site.

PLEASE BE AWARE that in SurveyMonkey you must fill out the entire application in one session. We strongly suggest that you refer to the print version of this application (available at http://www.library.ca.gov/services/libraries/COHS.html) to develop your answers before completing and submitting your application online. We have four application deadlines each year: March 10, June 10, September 10, and December 10.
