You have received this survey because you and your organization have used the services or resources provided by Connecting Ottawa. The data from this survey will be used to inform an evaluation project being conducted by CAP Consulting for the funder of the program, The Law Foundation of Ontario. Your feedback is extremely valuable to the evaluation of Connecting Ottawa - without your input we will not know the impact that Connecting Ottawa may be having for organizations and for clients.
We are requesting that surveys be completed by staff who have used the services or resources of Connecting Ottawa. If you have NOT used their services or resources, please send this link on to other staff in your organization who have used the service.
The survey will take approximately 10-15 minutes. Please try to complete the survey in one sitting; if you exit the survey and come back to it later, you cannot pick up where you left off. Participation in the survey is voluntary and confidential.
