Chapter Board Nomination Form 

You have been nominated to or would like to run for a Chapter Board position for the IL Continuity of Care Association. Before your nomination can be considered, please complete the questionnaire.
The nomination form should be completed by May 7, 2021. Voting will take place between May 17 - May 31st.  

Qualifications:
• Must be an association member for at least a year.
• Must be an active member who has paid all their dues by the end of the due period, October 31st.
• Must be in compliance with the Bylaws.

The current positions are available for the odd-year election. Terms are for two years.

President
Presides at all Association and State Board of Director’s meetings • Serves as a State Association Director on the Association's Board of Directors • Ensures that the resolutions and directives of the State Board of Directors are carried into effect except in those instances in which that responsibility is assigned to some other person by the State Board of Directors • Decides all questions of order, and shall have overall accountability for the Association’s fiscal affairs • Present an agenda report for each State Board of Directors Meeting
• Responsible for the investigation of all reports/concerns of officers failing to fulfill the duties and obligations of office.

Treasurer
The Treasurer shall be the principal accounting and financial officer of the corporation and shall serve as the State Board of Directors. He/she shall • Have charge of and be responsible for the maintenance of adequate books of account for the corporation • Have charge and custody of all funds and securities of the corporation, and be responsible therefore, and for the receipt and disbursement thereof • Receive, record, and issue receipt for membership dues received from the Chapters Treasurers • Prepare and present a written itemized report for all Board State Board of Directors meetings. Submit a written annual report to the Secretary no later than 30 days after the end of the fiscal year. • Perform other duties as the President or the State Board of Directors shall assign him/her.

By-Laws Chairperson
Serve as a State Association Director on the Association’s Board of Directors • Facilitate a committee of Chapter Bylaw Chairs whose primary purpose is to assure continuity and compliance with the purpose and objectives of the Association
• Communicate with Chapters Bylaw Chairpersons no less than annually in order to provide continuity and support • Prepare and present a report for each State Board of Directors Meeting • Submit a written annual report to the Association’s Virtual Secretary no later than 30 days after the end of the fiscal year • Perform other duties as the President or the State Board of Directors shall assign him/her.
