City of Bellevue Public Disclosure Survey

The City of Bellevue’s Public Records Division is seeking feedback from individuals who have received public records from us in the last 6 months. The survey consists of twelve questions about your overall experience and the customer service you received with the City’s Public Disclosure staff. It should take no more than 5 – 10 minutes to complete.

Our desire is to continuously improve the service we provide and to make Bellevue’s public disclosure process the best it can be to serve you. We will use your feedback to measure our performance and for insight on what areas we should focus on improving.

We will group responses to report them for City performance management documents; however, we will not publish or distribute responses for other purposes or identify individuals.
