10th Annual Texas Obesity Awareness Week

About The Texas Health Champion Award:

As part of the annual Texas Obesity Awareness Week, the Texas Health Champion Award will be presented to an individual and/or an organization who has demonstrated exemplary achievements in reducing the burden of obesity in Texas. The Texas Health Champion Award is a highly coveted state award given to individuals and/or organizations that have demonstrated a significant commitment to and impact on public health improvement in the area of obesity prevention and control. The individual and/or the organization receiving this award must have mobilized community efforts to prevent and reduce obesity and promoted physical activity and healthy eating through research, practice, policy development, social marketing, behavioral interventions or innovative partnerships.

Award Selection Criteria:

Awardees will be selected based on the following:

1.      Effective inclusion of essential populations (10 points)
2.      Sustainable program efforts (10 points)
3.      Impact on the burden of obesity (10 points)

The deadline for applications is June 17, 2017.

About Texas Obesity Awareness Week:

Texas Obesity Awareness Week was established in the 80th Legislative Session (2007) by House Bill 2313 and is designated to occur the second full week of September each year. The purpose of the week is to raise awareness of the health risks associated with obesity and to encourage Texans to achieve and maintain a healthy lifestyle. A highlight of the week-long campaign is the Texas Health Champion Award Ceremony, which is hosted by the Michael & Susan Dell Center for Healthy Living, part of The University of Texas School of Public Health Austin Regional Campus, and Live Smart Texas.

Applications can be submitted below:  Once you begin the online form, you must answer all the questions completely before closing your browser window, or your responses will be counted as incomplete.  We recommend typing the responses to the questions in a word document, then cutting and pasting your final responses into the nomination form.  Again, if you submit with incomplete sections, your answers will be lost.

The application includes 10 short answer/multiple choice questions. A short answer response to each question (list or essay-style) is required in order for the application to be considered.  

This application also also requires two (2) letters of support from people other than the nominee. The answers in this application cannot be substituted for a letter of support.A letter can provide further details of the nominee’s qualifications or additional examples of the nominee’s leadership, innovation, or future obesity prevention/awareness plans.  No more than two letters of support should be sent to Julie Latcham either via email (Julia.Latcham@uth.tmc.edu) or by mail:
