Introduction
Thank you for your participation in our survey that will help with our planning efforts in Los Angeles County!
Safe Med LA (Prescription Drug Abuse Coalition Los Angeles County), Community Education Action Team and the Los Angeles County Department of Public Health are collecting information around opinions on risk, availability/accessibility, consumption, and reasons why people use prescription medications non-medically as well as marijuana in your community. The information collected will help us with our planning efforts.
Please Note: All surveys will remain anonymous and all answers will be kept confidential. This means do not type your name anywhere on the survey; no one can identify you from the survey; and no one will be able to connect the answers to you personally. Some of the questions are personal, such as asking about drug use. Some people may find some of the questions uncomfortable or upsetting. You do not have to answer any question that you do not want to answer for any reason. Completing this survey is entirely voluntary, and whether you do or do not complete the survey will have no bearing on your ability to receive services from LA County or any cooperating prevention agencies. If you’d rather not do the survey, you are free to just leave this web page. Continuing to answer the questions indicates that you’re willing to participate in this anonymous survey.
