Consent Form

Aloha!

CTAHR's Food Systems Team - a multi-departmental working group - is conducting a survey to map CTAHR's faculty networks to assess and support a land-grant institution's capacity within local food systems. You are encouraged to take part in this network analysis study of the College. The findings will allow us to understand CTAHR's strengths and weaknesses, and help to define CTAHR's role. To effectively characterize CTAHR as a whole, we are aiming for a near 100% response rate. It will take 8-16 minutes to complete the survey, and we highly appreciate your participation. 

What am I being asked to do?
If you participate in this project, you will be asked to fill out a survey. You may also be invited to participate in an individual interview if you check “Yes” indicating your interest.

The short survey will consist of 22 questions in 3 sections: Consent Form, Background Questions, Collaboration Questions (Part 3), and Optional Feedback. The core eight survey questions in Part 3 will include questions that explore who you work with and how intensively you work with them. The survey may take about 8-16 minutes. 

Confidentiality and Privacy:
While your name is obtained, your response will be kept confidential. Names will be immediately coded and kept in an encrypted data file. No personal, identifiable information will be used in any analysis, reports, or data sharing.
