Cedars-Sinai CME Initial Planning Form

This initial planning form provides preliminary information to the Office of CME about the development of a new CME activity.  The purpose is to outline the educational rationale for doing the activity and identify intended outcomes for your learners.  Upon review and receipt, you will be contacted by the Office of CME and guided through the full CME application process. 

 

The CME Office requires a leadtime of 6-9 months for all CME course applications (full day, multi-day conferences) and 3 months for a new Regularly Scheduled Series (RSS).  All CME courses are subject to a CME accreditation fee.
