In order to participate in OC Public Libraries' (“OCPL”) virtual programming on Zoom, you will need to complete and submit this agreement to document your consent for you and/or your child to use, and be recorded on, Zoom.

If you are an adult participant, please complete part A of this agreement. If you are a parent or guardian of a child and you give permission for your child to participate in OCPL’s virtual programing on Zoom, complete part B of this form.
