Welcome to My Survey

Welcome to our Community Health Survey.  At Elkhorn Logan Valley Public Health Department (ELVPHD) we strive to make an everyday-difference in Burt, Cuming, Stanton and Madison Counties by:

·         Preventing the spread of disease,

·         Protecting against environmental hazards,

·         Reducing injuries,

·         Promoting and encouraging healthy behaviors,

·         Preparing for disasters through planning, and

·         Assuring quality and accessible health services.

Your input on this survey is important to us and provides us with information about the health of our communities that is not available from other sources. Your input will help us to identify key health needs throughout our four-county service area and will help guide our efforts and plans in the future. All who live, work or attend school in Burt, Cuming, Stanton and Madison Counties are invited to take this survey.

Our thanks to you… This survey will take about 25-30 minutes to complete. As a thank you, all adults over the age 19 and older who reside in the ELVPHD service area (Burt, Cuming, Stanton and Madison Counties) may enter a drawing for a $200 grocery gift certificate when you complete the survey.  One winner will be awarded in each county of our service area. Only one entry per household will be eligible for the drawing. Contact information will be required. The drawing will take place when the survey process is completed. We will attempt to contact the winners by phone and/or email. After three unsuccessful attempts to reach you, we will draw the next winner’s name. Winners of the prizes must consent to picking up the prize in person, having a photo taken and signing an image release as a condition of winning.

Confidentiality. We respect your privacy and your individual responses will be treated as confidential by Nebraska Association of Local Health Directors and Elkhorn Logan Valley Public Health Department. The data will be reported in summary only, with no individual answers identified. The data will be entered into the Nebraska Department of Health and Human Services Encounter Chart. Individuals who indicate that they are interested in receiving follow-up information or who are past due for selected health screenings based upon the current recommended guidelines will be contacted via email, text or phone by one of our public health nurses or educators to provide you with the information.
