2021 Mentor of the Year Nomination Form

The Mentor of the Year will be awarded by Camp HOPE America to a mentor who has displayed our mission and values through mentorship during Pathways year-round programming. The mentor is awarded the opportunity to choose one mentee who will receive a scholarship to be used toward college tuition. All submissions must be completed by October 1, 2021.
