Thank you for your time in filling out this CHW/Promotora Employer Survey. This survey is administered by the County of Sonoma’s Department of Health Services’ Community Health Workers for COVID Response and Resilient Communities (CCR) project in conjunction with Sonoma Connect | Sonoma Unidos (SC|SU), Center for Well Being (CWB), Redwood Community Health Coalition (RCHC), and Latino Service Providers (LSP). Data collected will be used to inform decision making around Community Health Worker programs and sustainability. There are ten questions total, and four we are required. Please share with other departments in your organization if they also have CHWs.  
 
For the purposes of this survey we will use the title of "Community Health Worker (CHW)" to refer to trusted community members who promote health and advance justice through direct care, community organizing, and many other roles. We recognize you might use different but similar titles such as Promotora de Salud, Peer Support Specialist, Community Outreach Specialist, and more, so we encourage you to fill out this survey if your staff or volunteers use these titles. We estimate this survey will take 5-10 minutes to complete.
