Thank you for agreeing to complete this survey! It is designed to gather information from young people about your personal experiences, the ways you cope, survive, and overcome different obstacles, and how these details may impact your health.

Here are some details that you should know:

1) Completing this survey is completely voluntary and you can withdraw at any point.

2) This survey may seem long but you should be able to answer everything quickly and easily. 

3) All information you provide will be completely anonymous and confidential. Anonymous means that we will not ask your name or any other information that can identify you. We will not be able to tell whose survey answers belong to who because no one from the research team will have any information about you. Confidential means that only members of the research team will have access to your survey responses.

4) If there are any items in the survey that you do not wish to answer, for whatever reason, please feel free to leave them blank.

5) If you need help from someone to complete this survey, you can ask whoever you feel safe and comfortable with. Just remember that if you do get help, that person will know what your survey responses are.

6) This survey and its accompanying research has been approved by the Research Ethics Board at the University of Alberta.
