Wildfire/COVID-19 CHA Survey

The purpose of this survey is to understand the effects of Wildfires and the COVID-19 Pandemic in Plumas County, CA to help Public Health respond effectively to future events. The survey will take about 5 minutes to complete.

At the end of the survey, you will be asked if you would like to participate in a raffle to win a $20 dollar gift card from Feather River Food Co-op. You only need to provide your contact information if you want to take part in the raffle. Contact information will only be used for the raffle drawing and will not be saved or used for anything else.
