The Massachusetts Developmental Disabilities Council (MDDC) is making a 5-year State Plan. This will include our goals, objectives, and planned activities for 2027- 2031.
We want to learn from members of the developmental disabilities (DD) community about what is important to include in the State Plan.
In this survey, you will:
- Answer questions about 4 different goals
- Pick your favorite goals
- Identify barriers to reaching your favorite goal
- Choose activities to help achieve your favorite goal
- Give us any other information you want to share that is important for people with developmental disabilities.
The questions will guide you through the survey.
Your participation in this survey is your choice. We estimate an average time of 10-20 minutes to complete the survey.
