The Office of Faculty Academic Advancement, Leadership and Inclusion is launching a celebratory initiative to acknowledge the diversity of our community and promote sharing, awareness, and learning.  We are seeking self-nominations and/or peer nominations of faculty members who are willing to share their stories.  In partnership, the SOM and the Provost Office have identified the below affinity months.  Nominations will be accepted on a periodic basis per the following schedule:

Nominations accepted September 9 – October 7, 2020
October: National Disability Employment Awareness Month
November: National Native American Heritage Month

Nominations accepted November 16 – January 4, 2021
February: Black History Month
March: Women’s History Month
April: Arab-American Heritage Month
Nominations accepted March 1 – April 16, 2021
May: National Asian American and Pacific Islander Heritage Month
June: LGBTQ+ Pride Month

Individuals nominated must agree to be publicly acknowledged and self-identify as part of the group being recognized.  If nomination is being made on behalf of a peer, please request their permission and agreement prior to completing the following form.
