General Survey Questions

The statements that follow are designed to assess common competing tensions within organizations.  For each of the statements, select from one of five options ranging from ‘Almost Never’ to ‘Almost Always.’ We intend to use the results of the survey to evaluate the general 'wellness' of the organization and, as appropriate, to plan future programs promoting enhanced 'organizational wellness.'
