CMMA Showcase: Performance Stories
Concordia University Wisconsin School of Pharmacy invites you to join a special, live event featuring the CMMA Performance Stories. Marking the end of a 12-month learning collaborative, participating organizations will showcase their most significant accomplishments to advance medication management systems.
 
Interested and engaged stakeholders are encouraged to attend and hear how a number of organizations are successfully integrating medication management services in primary care delivery for sustainable health care transformation. 
 
There is no limit to the number of individuals who may attend from one organization. CMMA teams presenting are encouraged to bring their full implementation team, including executive leadership.  
Each individual from an organization planning to attend the live event is requested to submit an RSVP.
