Informed Consent

* 1. In order to access the survey, please read the information below and check the appropriate conformation box.  If you agree to participate in this study, you will be able to access the survey.  Once begun, the online survey should take between 10 – 20 minutes to complete.

The goal of my doctoral dissertation research is to contribute to knowledge about stress, job motivation, commitment, and supervisor-member relationships it relates to project management and the CCPM method.  You are invited to participate in this research project because you are currently working in, or have recently completed, a project using CCPM.  

This study is a doctoral dissertation research project being conducted by Donald Humpert to partially fulfill the requirements for Doctor of Philosophy (PhD) in Industrial and Organizational Psychology at the University of Phoenix (UOPX) School of Advanced Studies (SAS).  If you have any questions about the research survey or your rights as a participant, you may contact the UOPX Institutional Review Board (IRB) office at (602) 713-7160 or via email at IRB@phoenix.edu.  This dissertation research is expected to be completed by early 2017.

You will not receive any direct benefits from participating in this study, but will contribute to the project management body of knowledge.  There are no inherent risks associated with participating in this research.  

Your participation in this research is voluntary.  You may choose not to participate.  If you decide to participate in this research survey, you may withdraw at any time.  If you choose to participate, you may decline to answer any question that you are not comfortable answering.  Choosing to participate or not to participate will not affect your employment or professional standing in any way.  Your decision to participate or not to participate will be kept confidential.  

To protect the confidentiality of your responses, only the Principal Investigator, Donald Humpert, will have access to the SurveyMonkey account being used to implement this survey.  This is an anonymous survey; neither your email address nor IP address will be displayed in collected data.  Your name will only appear on this informed consent page.  To keep your identity separate from your response data, the Principal Investigator has used the “sort” feature to create two views of the data collected: one showing only the pages with informed consent agreements, and another view showing only item responses with no identifying information.    

All data analysis will be conducted on a dataset exported from SurveyMonkey® with no personally identifying information to a temporary folder on the Principal Investigator’s hard drive which will be excluded from both physical and cloud-based backups.  No data on the hard drive can be accessed without entering the correct 15-digit password at login.  

During analysis, data will be able to be associated with a specific participant by respondent number.  Should you desire to be removed from the study at any time, your data will be deleted from the analysis dataset.  If you decide to opt out of the study after you have completed the survey, inform Donald Humpert of your decision via e-mail at don.humpert@email.phoenix.edu or telephonically at (443) 845-5352.
