Thank you for your interest in having your organization listed on the National Map of Community Care Hubs (CCHs), brought to you by the Center of Excellence to Align Health and Social Care (COE), part of the Aging and Disability Business Institute at USAging. The National Map of CCHs can be found here: https://coe.aginganddisabilitybusinessinstitute.org/national-cch-map/

To be considered for inclusion on the map, interested organizations must:
  • Operate as a CCH according to the updated definition of a CCH published by the COE
  • Complete the Submission Request Form. Click the “Next” button below to begin.
All submission requests will be reviewed by the COE, and you may be contacted for additional information. The COE will notify you via email if your submission is accepted.

INSTRUCTIONS
Please review the following brief instructions prior to beginning the Submission Request Form:
  1. Submit only one response per CCH.
  2. When completing the request form, please indicate service information for your CCHs primary service area only.
  3. If your CCH operates in an additional region(s), state(s), or territory(ies) outside of its primary service area, you will be instructed to click on a link at the end of the request form to provide additional multistate information for all other areas served outside of your primary service area.
  4. List all counties served regardless of if your service area includes the entire county.
  5. Please include all of the services your CCH offers even if they are not all available in every county in your primary service area.
  6. Do not include services that are offered by CBOs in the CCHs network of providers if that service is not available for contracting through the CCH.
