For the safety of all of our members and guests, all individuals entering the Columbus Curling Club must be fully vaccinated against COVID-19 (with a few exceptions).
Effective August 8th, 2021, all individuals entering the Columbus Curling Club must be fully vaccinated against COVID-19.
Exceptions to the vaccination requirement are reserved for the following groups:
-Individuals ineligible based on the FDA’s Emergency Use Authorization or Approval Status (ex: children under 12 years old)
-Individuals with a note from a medical professional (see exemption form)
-Contractors hired by CCC to perform work
-Vendors making deliveries
Unless previously submitted/provided to Columbus Curing Club, individuals should be prepared to show their COVID-19 Vaccination card or an exemption form signed by an appropriate medical professional.
Mask Policy
All individuals inside theColumbus Curling Club must wear a face covering when more than 1 person is at the club. Face coverings must fit appropriately and cover the nose, mouth, and chin completely. Face coverings with vents are prohibited. Food and drink beyond taking a drink of water are prohibited for individuals required by this policy to wear a mask.

Use of this online submission form is OPTIONAL.
For the convenience of our members and guests, we have enabled this online portal for you to submit your proof of vaccination or documentation of an exemption online. You are NOT required to use this form. Alternatively, you can show your proof of vaccination or documentation in-person to a designated CCC member. Guests, you can do this when you arrive for your event
Your submission will be deleted after it has been reviewed by a member of our governance team. We do NOT store the details of your vaccination or exemption documentation.
