TEAM NOMINATION FORM
If you're wanting to enter a team into the Canterbury Bankstown Futsal Competition at the Morris Iemma Indoor Sports Center (MIISC) - please complete the below questions/team nomination form. You will also find below a brief breakdown of necessary competition information.
COMPETITION STRUCTURE:
Monday
Under 9 Boys (minimum 7, max 9 years of age as at 31st December 2017)
Under 9 Boys (minimum 7, max 9 years of age as at 31st December 2017)
Under 9 Girls (minimum 7, max 9 years of age as at 31st December 2017)
Under 11 Boys (minimum 9, max 11 years of age as at 31st December 2017)
Under 11 Girls (minimum 9, max 11 years of age as at 31st December 2017)
All Aged Women/All Aged Men (minimum 17 years of age as at 31st December 2017)
All Aged Women/All Aged Men (minimum 17 years of age as at 31st December 2017)
Wednesday
Under 13 Boys (minimum 11, max 13 years of age as at 31st December 2017)
Under 13 Girls (minimum 11, max 13 years of age as at 31st December 2017)
Under 15 Boys (minimum 13, max 15 years of age as at 31st December 2017)
Under 15 Girls (minimum 13, max 15 years of age as at 31st December 2017)
All Aged Mixed (minimum 17 years of age as at 31st December 2017)
- All games are 5 v 5.
- A maximum of five (5) players on the court at any one time including the Goalkeeper.
- A maximum of 10 players can be nominated for each game.
FIXTURES:
12 week program:
Week one - Week commencing Monday 4th September
Week twelve - Week commencing Monday 4th December
Finals - Week commencing Monday 11th December

Individuals:
Juniors (age 11-17):
Registered players (2017 winter)* - $45.00
Unregistered players (2017 winter) - $50.00
*These are players who are registered in 2017 for the outdoor winter competition.
Seniors (18+):
Registered players (2017 winter)* - $80.00
Unregistered players (2017 winter) - $110.00
Teams:
Team registration fee (per team, per competition): Juniors: $100.00, Seniors: $150.00
Match fees (per team, per match played) - Juniors: $35.00, Seniors: $70.00
