CARES Relief Application

DEADLINE TO SUBMIT, DECEMBER 28, 2020

The CARES fund is designed to alleviate financial hardship for those businesses impacted by the Coronavirus pandemic. Funds will be administered to businesses within the Cities of Woodbine, who have an outstanding mortgage payment, rental arrears and/or utility expenses from March 2020 to current.

Please complete all fields within this application that are applicable. Any field indicated by an asterisk (*) must be completed for the application to be considered. If NOT ABLE to upload documents, please call (912).882.7279 or email mfrancis@camdenconnection.org after finishing all possible questions, to schedule a drop off of forms. A legible cellphone picture is acceptable.

After completing the application, you will receive an email notification.

Note that a $5,000 maximum assistance limit is imposed per eligible business for the first phase of program aid. If a bill requested is above such limit, applicant should be able to prove the balance of the bill can be met through another source.

**The business address must be within the city limits of Woodbine. It is possible to have a postal address marked as "Woodbine" without being within city limits, we can verify this information upon review if unsure.
