The purpose of the survey is to follow up on the funding sent to child care providers to support expenses related to the COVID-19 pandemic.

A response to this survey is mandatory for anyone who received funding after August 20, 2020. The State of Montana is required to report on the success of the CARES Act funding under CFDA#21.019.
