CAMHPRO Quarterly Survey

If you have received this survey, we thank you for attending one or more of CAMHPRO’s Peer Action League trainings, activities or events from June 1-September 30 of this year. These activities are funded by the U. S. Substance Abuse & Mental Health Services Administration’s (SAMHSA) Statewide Consumer Network grant. A crucial requirement of this grant is to track participant and group outcomes resulting from CAMHPRO activities. At the end of each quarter CAMHPRO respectfully requests that you complete a survey for us to gather the required information needed for this grant. The results are completely anonymous. The accuracy of these outcomes is dependent on your answers four times each year. However this is voluntary and if you choose not to provide this information we are still happy to provide you with our services.
