Who is CACHC?
The Canadian Association of Community Health Centres (CACHC) represents Community Health Centres (CHCs) across Canada and works to improve access to community-centred, inter-sectoral and team-based primary health care. 
 
Why this survey?
CACHC is developing a position paper and recommendations to the federal government regarding the role and value of CHCs, outlining why federal investment in CHCs -- including earmarked funding transfers to the provinces/territories -- is critical to improve health and wellbeing and to fulfill the federal government's mandate.
 
This survey will assist CACHC to better demonstrate the multi-sector work of CHCs and enable various federal government departments to better understand how CHCs support their health-related mandates. It will also help CACHC to document gaps in federal and provincial policy and funding, and enable us to identify opportunities for investment in existing and new CHCs across Canada. 
 
WE STRONGLY ENCOURAGE YOU TO PARTICIPATE IF YOU REPRESENT AN EXISTING CHC OR A GROUP LOOKING TO ESTABLISH A NEW CHC. We ask for no more than 1 (one) submission per CHC please.

For the purposes of this survey, a "Community Health Centre (CHC)" is any not-for-profit corporation, co-operative, or government agency which satisfies the following five criteria:
 
  1. Provides inter-professional primary care through physicians, nurses, nurse practitioners, social workers and/or other providers.
  2. Integrates those primary care services with health promotion and community health programs/services.
  3. Involves members of the community in identifying service priorities, evaluating services, and/or governing the organization.
  4. Actively addresses the social determinants of health through programs, partnerships, and/or advocacy initiatives that seek to improve conditions for health such as access to affordable housing, reducing poverty, and other factors affecting health.
  5. Demonstrates commitment to health equity and social justice, including working to improve access to services and supports for members of society that are marginalized and/or face significant barriers to health and accessing appropriate healthcare.

The survey takes approximately 15 minutes. Deadline is August 26, 2016.
