CAMP Membership Sign Up - $150 each month

Welcome to Seniors Creating Art CAMP information page and sign up

Seniors Creating Art Creative Art Making Program, CAMP, is the perfect way to give your residents access to the amazing health and wellness benefits that participating in creating art provides. After completing this Sign Up Form, be on the lookout for a call/email from Seniors Creating Art staff who will help finalize the details for your CAMP workshops! Please allow at least two weeks from sign up date to begin your first workshop. Thank you!
1.What is the name of the Community signing up for CAMP
2.What is the address of the Community
3.What is the name, phone number AND email of the contact person for your CAMP workshops? 
4.Ideally, your monthly CAMP workshop will take place the same week of each month. For example: the 1st Wednesday of each month or the 3rd Thursdays of each month. Keeping that in mind, enter your #1 preferred Day of the week for your monthly art workshop
5.CAMP workshops are 1 hour long. Enter your #1 preferred time - morning/afternoon/specific time (e.g. 3pm or mornings)

8.Your CAMP workshops will switch mediums many times throughout the year. From painting, to drawing, to clay sculpting, cardmaking, your residents will get to experience art in new ways each month! Below are some of the most popular mediums to start off your CAMP. Please pick a medium you would like to start with, or if you like to be surprised, your Art Instructor will come
prepared with a project of their choice for your group!
9.BILLING INFORMATION - You will be billed $150.00 on the 30th of each month for your CAMP workshop for that month. How would you like to pay for your monthly workshop? If you select Online Payment, you will have the option to leave this site and go to our payment platform and sign up to be automatically billed $150.00 each month.
