Step 2: Provide a Nomination Statement and Photo

Please provide some details about why you're running for Council and highlight where possible, your governance or relevant experience, your leadership skills and knowledge of the healthcare system (maximum 400 words).

Step 3: Complete the Candidate Consent, Nomination Form and Conflict of Interest Form

Download the PDF, complete the form and upload the finished version.

Step 4: Upload a Current Resume

Please provide a current resume.
