ADPH Awards 2024 Nomination Form

We are delighted to be launching the ADPH Awards 2024, which will once again be held in a ceremony at our Annual Conference in November. If you would like to nominate a Director of Public Health and/or their public health team for an award, please use this form.

Question Title

* To make a nomination for an award, please select a maximum of two of the below options:

CMOs' National Impact Award - for a DPH in recognition of the impact they have had since starting the role. This award is supported by the CMOs of all four nations.

Innovative Practice Award - for a DPH who has demonstrated or championed innovative practice within their organisation.

Partnership Award - for a DPH who has led a successful local or national partnership with a sector or organisation.

Mentoring Award - for an ADPH mentor who has shown dedication and commitment to mentoring as part of the ADPH mentoring scheme.

Practice Improvement Award - for a public health team which has established effective ways of improving public health practice.

Team Award - for a public health team which has worked exceptionally well together to improve public health outcomes in their area.

Research Award - for a DPH and their team for undertaking public health research, or using research evidence, to inform decision making in their organisation.
