Client Survey

* 1. BirthChoice Dallas would like to get your feedback and has asked a team of MBA students at the University of Dallas to conduct this survey. 

Purpose: We are interested in collecting your perceptions of this clinic.
 
Description of Procedures: This research study will take place in an online survey, and it is estimated to take about 5 minutes of your time. Your responses will be anonymous.

Confidentiality: Your survey answers will be sent to a link at SurveyMonkey.com where data collected is password protected. No one, other than the researcher will have access to the collected individual data. Your records will be kept anonymous.
 
Voluntary Participation: Your decision to participate in this survey is entirely voluntary. You may withdraw from the survey at any time without any penalty.
