Be the Village Campaign Evaluation - Healthcare and Service Provider Survey

The following information ensures informed consent for respondents.

Purpose of Survey
  • To gather insights from healthcare/service providers about their experiences with and perspectives on the Be the Village campaign.
  • To identify opportunities to strengthen FASD prevention messaging, improve resources, and better support healthy pregnancies, recognizing the role of everyone involved.

What We Are Asking of Participants
  • If you are a healthcare or service provider, you are invited to complete a short online survey that should take approximately 10 minutes.
  • Participation is entirely voluntary. You may choose not to answer any question or stop the survey at any time before submitting it.

Compensation

As a thank you for your time, participants will receive a special code to order Saskatchewan Prevention Institute resources with shipping costs waived until June 30, 2026.

Confidentiality
  • All survey responses are anonymous; no names or identifying information will be linked to your answers.
  • Completed survey data will only be accessible to the evaluation team.
  • A summary report will be created, and no identifying information will be included.
