You are invited to participate in a research project about sexually transmitted diseases (STDs) in Bell County. Questions will be about sexual risk behaviors, sexual history, and basic demographic information. This survey is anonymous which means no information will be asked which can identify an individual. In the event that you have any questions please contact Bell County Public Health District at 254-773-4457. Once Submitted the survey cannot be withdrawn. Any partially completed surveys will be deleted and the data not submitted for inclusion - so please answer each question asked. You may stop at anytime you want, but if you do not complete the survey, the answers you have provided will be deleted and not included. Participation is voluntary, and responses will be kept anonymous. All data is protected and stored by survey monkey (information about their data protection procedures can be found here:https//www.surveymonkey.com/my/take-a-tour) and analysis will be conducted on Health district computers which are password protected.
