WELCOME to the THOUSAND OAKS INSTRUMENTAL MUSIC DEPARTMENT!!!

We are happy to have you join our program.

The information on this questionnaire will be used by the Band Director, Staff and Boosters
through Charms email communication and printed form as needed by the Music Department.

PLEASE NOTE: This questionnaire is for the Instrumental Music Department use only. School registration and course selection must be completed with the TOHS counseling department.

DON'T FORGET TO ATTEND THE PARENT MANDATORY ORIENTATION MEETING IS ON Wednesday, April 24, 2019 -6:30 pm for NEW member Parents AND for Returning Member parents in the TOHS CAFETERIA  (MPR)

This Orientation Meeting is MANDATORY for ALL Parents of ALL MUSIC DEPARTMENT STUDENTS and Optional for students to attend. (Music Department Consists of Brass/Woodwind/Jazz/Percussion/ColorGuard)
This orientation provides you with information on core classes in the department as well as the "optional performance ensembles".   Time Commitment, Calendar of Events, Funding, Boostering, and Fundraising are topics covered and Mr. Martone will discuss all facets of the TOHS Instrumental Music program. The mandatory meeting will include instructions on placement Auditions.

There will be forms that will need to be completed and turned in at the conclusion of the meeting.  These forms are needed for audition scheduling.
