The Summit County Continuum of Care is offering this survey to youth aged 15-24 that are experiencing or at risk of becoming homeless. The point of this survey is to understand the housing and support needs of the youth 15-24 in our community. Please know that the information collected will be used for statistical purposes only, and should be filled out once. No personal information will be collected and any information that is shared will be kept anonymous and confidential. If there are questions that you are uncomfortable answering please leave them blank. If there is a concern for the safety of the individual that is doing the survey, these concerns will be reported to the proper authority.
