The 2023 Health Equity Symposium Community Voices Survey

Informed Consent

This survey is designed specifically for birthing people and people of birthing age (18 or older) who access reproductive healthcare services on Cape Cod. You will be asked to answer questions about your identity and the types of reproductive healthcare services you have used, such as family planning and sexual health services, pregnancy, birthing, and postpartum services, and mental health services. You will also be asked about your ability to access these services. Information collected in this survey will be used to inform questions in our focus groups, discussion topics and resources for The 2023 Health Equity Symposium, and potential future projects to increase reproductive healthcare accessibility. 

Your participation is completely voluntary, and your right to confidentiality will be upheld. Your name will not be associated with any of the results. Anytime you share information online there are risks such as hacking, but we are collecting this information on a secure system. For more information on this system, click here. Any additional data collected through this system, such as names, IP addresses and location, will be deleted from the raw results. Only the members of Cape Cod Voices, Massachusetts Mom Advocates, and Belonging Books will be able to see the raw data from this survey. Quotes and paraphrasing of findings from the survey may be used during the symposium but will be done so anonymously.

We understand talking about reproductive healthcare, or associated experiences, can be difficult and this survey may put you at risk for emotional distress. We will not ask you about any specific details about your experiences but if you become uncomfortable or do not want to answer a question, feel free to not answer or you can close out. 

Depending on your experiences, we expect this survey will take about 5-8 minutes to complete.
