Question Title

* 1. OVERVIEW

Governors State University Center for Performing Arts (GSU CPA) is committed to presenting a safe online learning environment for all camp participants.  Camp will be held entirely remotely, and will be a mix of online teacher instruction and offline work. Camp will meet online daily both as a group and in small teams, and may live stream events throughout the week via Zoom and Band.Us. All experience levels welcome.  Camp is for Ages 9 - 15 and will be from 10a-3p Monday - Friday (five weekday sessions.)  Cost is $125 per week for each camper.  ALL SALES ARE FINAL.  There are NO REFUNDS OR CANCELLATIONS after registration is complete.

 Participation requires a smart device with the Zoom and Band.Us Applications downloaded, and may require campers to independently purchase basic crafting and school supplies for use in their house.  

Prior to the first session, GSU CPA will contact parents/guardians with an emailed link for a brief (online) Group Orientation Session to share the camp's objectives and expectations, to verify home equipment functionality for the media platforms, and to access the daily itinerary for the program. Camp counselors will share an age-appropriate internet safety plan at this online orientation session.  Parental guardians are encouraged to assist children with online programming. 

During virtual sessions, camp participants may see and hear anything within the counselor's or other students' camera's audio and viewing areas. Please make sure all people in the participant's household understand this prior to the online sessions. Each virtual session may be recorded, including audio/visual interactions from camper's home computer or device, and including images and audio of camp participants. Counselors will generally mute participants during group instruction, but may unmute to ask/answer questions, participate in group discussions, etc. If participants act disruptively or share inappropriate audio or video content during any online camp session, they may be removed from the online activity.  If a participant is removed, the counselor will contact the parent to discuss the occurrence.

* 2. PARTICIPANT PHOTO / VIDEO / STREAMING RELEASE         

I hereby authorize GOVERNORS STATE UNIVERSITY to film, tape, or otherwise record my or my minor child camp participant's likeness, image, voice, interview and/or performance for the CPA produced and presented “virtual children’s summer camp program” to be held entirely online via ZOOM and/or other social media platforms.  I also authorize GOVERNORS STATE UNIVERSITY to use and authorize others to use this recording, videotape or film, or any part thereof, on an irrevocable and unlimited basis in all markets and media now known or hereafter devised, including online streaming.  I additionally authorize GOVERNORS STATE UNIVERSITY to use my or my minor child camp participant's likeness, voice, performance, and biography for both archival purposes and to publicize or promote future programs.

* 3. PARENT/GUARDIAN AUTHORIZATION & WAIVER OF LIABILITY             

Please be aware that in registering yourself and your minor child/ward for participation in this Virtual Summer Camp program presented by GSU CPA, you, for yourself and as the parent or legal guardian of the child named above, will be waiving and releasing all claims for injuries that you and your child might sustain arising out of or in connection with this program.  GSU Center and its contracted camp counselors will never be in physical contact or proximity whatsoever with virtual camp participants.  The entire camp will be conducted online only, with all campers participating at a remote location determined by their parents/guardians choice.

* 4. PERMISSIONS ACKNOWLEDGEMENT

I hereby give permission for my child to participate in all virtual camp programming and activities. I recognize and acknowledge that my camper will at all times be under my parental/guardian supervision and that all camp activities will be presented remotely via internet media platforms. I agree to assume the full risk of all injuries, damages, or losses regardless of severity which I or my child or ward may sustain as result of participating in any activities connected or associated with any such programs and I waive and relinquish all claims I or my child or ward may have against Governors State University, the Center for Performing Arts, and its officers, directors, employees, agents, contractors, and volunteers as a result of participating in the activities and programs in connection with virtual summer camp. 

 I, on behalf of my child, release, indemnify and hold harmless GSU Center from and against any and all liability, actions, debts, claims and demands of every kind whatsoever, specifically including, but not limited to, any claim for negligence or negligent acts or omissions and any present or future claim, loss or liability for any type of injury to person or property that my child may suffer, for which my child may be liable to any other person, or that may or does arise out of my child’s participation in the Event.  Furthermore, I acknowledge that I have received, read, and understand camp policies, procedures, code of conduct, and video/photo/media release forms, and agree to abide by them.  I hereby grant the permissions stated therein.

This RELEASE contains the entire agreement between the parties and the terms of this RELEASE are contractual and not a mere recital.  I have been given ample opportunity to read this document and I understand and agree to all of its terms and conditions. I understand that I am giving up substantial rights (including my right to sue), and acknowledge that I am signing this document freely and voluntarily, and intend by my signature to provide a complete and unconditional release of all liability to the greatest extent allowed by law.

 My electronic signature on this document is intended to bind not only myself but also my successors, heirs, representatives, administrators, and assigns.
